Who are we?

‘Dr Mountis Skin Clinic’ is the trading name of ‘P MOUNTIS LTD’ . For the purpose of the Data Protection Act 1998 (the Act) and General Data Protection Regulation (GDPR) 2018, the data controller is P MOUNTIS LTD. For simplicity, “we”, “our” and “us” refers to ‘Dr Mountis Skin Clinic’.

What data do we collect?

We only collect data that is relevant to the service that Dr Mountis Skin Clinic provides. This includes, but isn’t limited to: your name, date of birth, postal address, email address, telephone number and medical history

How will we use your data?

Your personal and medical information is used to process your order and to ensure that any treatments you receive are safe and appropriate for you. It’s also used for lawful medical record keeping as well as to communicate with you regarding services we provide.

How do we store your data?

This is stored electronically, with password encryption, and/or on paper-based records that are stored securely. Only select 3rd party companies will have access to your personal data, and only where necessary to provide you with the services you receive at Dr Mountis Skin Clinic, such as pharmacists for prescription-based treatments, and calendar booking services in order to hold your appointment information.

How long do we keep your data?

As part of lawful medical record keeping, your data is stored for a minimum of 7 years after your last treatment or contact with Dr Mountis Skin Clinic, after which time it may be destroyed. Your information we use for consented marketing purposes will be kept with us until you notify us that you no longer wish to receive this information.

What are your data protection rights?

You have the right to request a copy of any personal information we hold about you. This may be subject to a fee of £10 to meet our administration costs, and will be provided to you within 14 days.If at any point you believe the information we process on you is incorrect, or you wish to request to see this information and even have it corrected or deleted, please contact us by emailing at pmountis@doctors.org.uk.
