General Clinic Terms and Conditions
Please read these Terms and Conditions before using www.pmountis.com operated by Dr Mountis Skin Clinic (“us”, “we”, or “our”). By using this website (www.pmountis.com), you
agree to be bound by the following terms and conditions.If you do not agree to comply with and be bound by these Terms and Conditions, please stop using our website immediately.
General Information:
Patients that are new to our practice are required to have an initial consultation before any treatment or service can be provided.
All patients are required to provide personal details including name, date of birth, address, telephone and email, in order to secure an appointment.
All patients are required to complete a secure online medical history questionnaire prior to their first appointment and at each subsequent appointment if there are any changes.
This will be sent to patients prior to their appointment and is essential for patient safety.
All information will be treated as confidential and protected in accordance with Data Protection legislation.
Photographs:
At each appointment you will be required to provide written consent for Dr Mountis to take and store before and after photographs of each treatment area. This is a mandatory requirement to receive treatment at Dr Mountis Skin Clinic. It is not mandatory to consent
to the sharing of your photographs and we will seek separate written consent from you if we would like to use your photographs for anything outside of your medical aesthetic record, of which you can decline to consent to.
Review appointments:
Routine review appointments are offered after certain treatments, including Botulinum Toxin and Dermal Fillers, as a courtesy. If you are unable to attend the review appointment, no treatment adjustments can be provided once the review period of 2-3 weeks has passed.
Treatment Results:
Dr Mountis will discuss treatment expectations at your consultation and will strive to achieve the best results for you. However, treatment results may vary from person to person and no guarantee of an exact result can be provided. This can be due to various reasons including individual skin types and lifestyle.
Booking Appointments:
You can request an appointment on our website. Alternatively, you can email Dr Mountis
at pmountis@doctors.org.uk. There is a booking fee of £100 to secure your appointment
for injectable treatments (anti-wrinkle, dermal fillers, lip fillers and skin boosters), which is
redeemable towards the cost of treatment(s) performed within 3 months of the appointment. The preferred payment method is by BACS.
Payment:
Our Terms are full payment on the day of treatment directly in the clinic. Cash and all
major credit and debit cards are accepted, including American Express. We do not accept cheques. Your credit/debit card details will be processed directly through a Barclaycard Flex Terminal and will not be held within our booking system.
Refunds:
Whilst we always aim to provide excellent service, we cannot guarantee your results and cannot offer refunds if the results achieved fail to meet your expectations.
Appointment cancellations and rescheduling policy:
Failure to cancel or reschedule within 24 hours prior to your appointment will result in the loss of your £100 booking fee. You will need to pay a consultation fee again should you wish to re-book the appointment.
No show/late arrival policy
If you fail to attend a Consultation appointment (No Show) for any reason without any notification, you will forfeit the £100 held on account and it will be processed to pay for wasted appointment time. You will need to pay a consultation fee again should you wish to re-book this appointment.
If you are more than 10 minutes late, we may be unable to see you, and therefore need to
reschedule your appointment.
Child Policy:
We do not treat children or young adults under the age of 18. Please do not bring children to the clinic under any circumstances. We regret that we are unable to allow children to remain unaccompanied in the reception area.
Policy on pets:
We regret that we are unable to allow pets in the clinic.
